Travel Vaccinationsglobal air travel

If you require holiday vaccinations for foreign travel you must complete a Travel Questionnaire prior to attending for vaccination.  You can print off the Questionnaire from the link below and return to the Surgery as soon as possible or, alternatively, you can complete the Questionnaire on-line.

It is imperative that you complete the Questionnaire at least 6 weeks before you travel.  You will need to have your vaccinations at least two weeks prior to travelling to ensure that the vaccine is effective.  If Questionnaires are returned in less than 6 weeks the Practice cannot guarantee that you will be vaccinated and it may be necessary for you to go privately which could incur additional costs. 

Some travel vaccines provided by the Surgery are ordered on a Private Prescription and incur a charge over and above the normal prescription charge as not all travel vaccinations are included in the services provided by the NHS.
