The NHS in England is introducing the Summary Care Record, which will be used for emergency care.  The record will contain information about any medication you are taking, allergies you suffer from and any bad reactions to medicines you have had.  This is to ensure those caring for you have enough information to treat you safely.  Your Summary Care Record will be available to authorised Health Care staff providing you care anywhere in England, but they will ask your permission before they look at your records.  This means that if you had an accident or become ill, Health Care staff treating you will have immediate access to important information about your health. 

 

This Practice supports Summary Care Records and as a patient you have a choice:

 

Ø  Yes : I would like a Summary Care Record – you do not need to do anything and a Summary Care Record will be created for you.

 

Ø  No : I do not want a Summary Care Record – Please see below:

 

Enhanced Data Sharing Model (EDSM)

 

Currently electronic records are maintained in all of the organisations that you receive health care.  These organisations can only share information from your medical records via letter, email, fax or phone.  Sometimes this can delay treatment.  Your Practice uses a computer system (Systmone) that allows access to sharing full electronic records across a variety of health care organisations.

 

This Practice supports Enhanced Data Sharing and as a patient you have a choice.

 

Ø  Yes :  You can choose to share your electronic records with other care services – you do not need to do anything.

 

Ø  No :  You can choose not to share you electronic records with other care services – Please see below

It is essential that Patients decide whether they wish to share their medical information with other health care professionals. 

   

You have the choice to:

 

·         Share your data for both Summary Care Record and Enhanced Data Sharing

 

·         Choice to OPT OUT of both of the above

 

·         Choice to share your data for one of the above and OPT OUT of the other

 

To avoid any potential difficulties with your medical records being accessed inappropriately it would be preferred if you agree to either allowing your data to be shared for Summary Care Record and Enhanced Data Sharing rather than your data being shared with one of the above and opting out of the other.  However, this is your choice and your medical records will be marked according to which options you choose.
