Patient On-line and Record Access - ‘It’s Your Choice’
You can now book appointments with a GP, request Repeat Prescriptions for currently prescribed medication (except when a medical review is required) and look at our Summary Record on line.
The benefit is being able to manage your medical condition better and accessing your medical record anywhere in the world should you need medical treatment whilst abroad.
The decision to join this scheme is voluntary and you may withdraw from it at any time if you so wish.
On joining the scheme you will be given login details and you will need to provide your own unique password to ensure that only you are able to access your medical record unless you choose to share your details with a family member. It is your responsibility to keep your login and password details secure. If you know or suspect that someone has gained access to these details you must change your password immediately. If you are unable to do this please contact the Surgery and request that your online access is removed with immediate effect. It is not recommended that any part of your record is printed off for this reason.
The Practice reserves the right to remove online access to anyone who does not use their access rights appropriately.
Things to consider before applying for online access to your record:
Forgotten history: There may be something you have forgotten in your record that you may find upsetting
- Choosing to share your information with someone: This is your decision and your responsibility to keep the information safe and secure
- Coercion: if you think you may be pressures into revealing details from your Summary Care Record to someone else it may be preferable that you do not have access at this time
- Misunderstood information: Some of the information in your record may be highly technical and written by Specialists and not easily understood. Please contact the Surgery if you require clarification
- Abnormal results and bad news: There is a risk that you may read something you may find upsetting

If you would like to register for Patient On-line: Report Access please call in at the Surgery and ask the Receptionist for a registration form. The Receptionist will explain how the system works and give you all the relevant information you require to register.
