Patient Participation Group Annual Report and Action Plan March 2015
The Practice is required to submit information to NHS England with regards to the development and monitoring of the Patient Participation Group. The report must include the following information:
- the make-up of the PPG against the Practice population, how often the group meets and how it engages with the Practice population to routinely capture feedback
- feedback received and reviewed by the Patient Participation Group
- the three key priority areas identified for improvement by the Practice and the Patient Participation Group
- the actions which were taken to address the priority areas referred to in the (iii) and the resulting changes made.
