ADHD/Autism
You have contacted the surgery to have an assessment for Autism and/or ADHD either for yourself or your child.
Prior to the referral appointment with a GP please can you fill in the appropriate forms and then book a GP appointment to complete the referral. Please DO NOT book an appointment until all the required forms including school reports are available.
Children – Pre-school or in Primary school (up to end of school year 6)
If there are concerns about neurodevelopment disorders (ASD, ADHD) the GP will need nursery/ school information to accompany your child's referral.
Only children above the age of 2 in nursery or primary school can be referred for an autism assessment. Below this age book a GP appointment to discuss potential other causes of delayed development/ referral.

Triage and Navigation provides access to emotional and mental health services for Children and Young People up to the age of 18 registered with an LLR GP.
If there are concerns about neurodevelopment disorders (ASD, ADHD) the GP will need school information to accompany your child's referral.

Right to Choose Pathway (Adult and child)
The current waiting time for an assessment on the NHS is now in excess of three years. Patients are able to exercise their right to be assessed elsewhere using the Right to Choose Legislation.
Patients are able to research this alternative pathway, complete the necessary paperwork and return to GP for formal referral.
Right to Choose is the umbrella name for a multitude of agencies who are able to offer their services and allows patients to find a service that is right for them. Services currently being used by some of our patients include ADHD 360, Problem Shared, Clinical Partners, Psicon, Provide Wellbeing, Evolve Psychology and we believe Psychiatry UK may have opened their doors again recently after a spell of being full to capacity. Please also visit the ADHD UK website on https://adhduk.co.uk/right-to-choose/ where you will find the latest Right to Choose Providers and wait times.
These websites will advise you as to what assessments they offer, ie, ADHD, Autism or both, whether it caters for children, whether this is just a one-off assessment to provide a diagnosis only, or if follow-up/management/medication is available. It will also advise whether this is a virtual assessment or face-to-face which may require travelling a distance.
Unfortunately, as this is a relatively new pathway used by the Surgery, we have not had the capacity to explore all these services individually to be able to recommend any one in particular that will suit your needs and what they offer in terms of ADHD and/or Autism assessments and which ones cater for children, but the websites will provide all this information.
Please have a look at all the available services on offer and follow any instructions for downloading and completing letters, questionnaires etc and return to your GP who will then arrange formal GP referral if required.
Please complete the relevant form prior to your routine GP appointment. Please note the reception team have been asked by the GPs that only Routine Pre-bookable appointments are booked for the above. An Urgent on the day appointment is an inappropriate request.
